​Here is a list of procedures and codes that are done in our office.  Please call your insurance company to see what your responsibility may be if the doctor feels any testing is indicated and you choose to have done:

When you call your insurance company, you will need to ask them if these procedures are a covered service. Please give them our TAX ID#232916058 as well as our NPI#1952364739.  If these procedures are a covered service, you will want to find out what your responsibility may be.  You may have deductibles, copays, or coinsurance that will be your responsibility to pay.  If after speaking to your insurance company you have any additional questions, please call Kathy, our billing manager at (215)968-6000.

CO PAYS FOR OFFICE VISITS AND INJECTIONS ARE EXPECTED AT THE TIME OF VISIT.

ONCE DEDUCTIBLE HAS BEEN DETERMINED, WE EXPECT PAYMENT TO BE PAID IN FULL UPON REQUEST OR RECEIPT OF STATEMENT.

ONCE CO INSURANCE HAS BEEN DETERMINED, WE EXPECT PAYMENT TO BE PAID IN FULL UPON REQUEST OR RECEIPT OF STATEMENT.

IF A PAYMENT PLAN IS REQUESTED, PLEASE ASK TO SPEAK TO KATHY, OUR OFFICE MANAGER AT 215-968-6000 AND SHE WILL SET UP ARRANGEMENTS.
