On Call:
For true medical emergencies, please call 911 and go to the nearest Emergency Department. If you are a current patient and require urgent assistance after hours or on weekends, please call our office and our call service will contact the on call provider.
Refills:
We require our patients be seen in our office regularly as requested by our providers to allow refills of medications. This ensures we are providing medications that are still appropriate, without any adverse effects and to determine if it should be continued. Some medications require closer monitoring. You may notify your pharmacy to prompt a refill request but kindly provide 72 hours to process.
Bill payment/cancellations:
Please be assured that we make every effort to provide high-level of professional care in a cost-effective manner. If you have a co-pay, co-insurance this will be collected at the time of office visits or for allergy shot visits. We request that you pay your balances once insurance claims are process upon receipt of your statement. If you unable to pay in full at time of receipt statement, please contact our Billing Manager, Libby.
While we understand circumstances arise that may require you to cancel your or your child’s appointment in a short notice, to better serve our patients, there will be a cancellation fee of $70 for follow up visit and $125 for new patient visit. The fee applies for no-show to appointments as well. Your insurance will not cover this fee. If you need to cancel an appointment, please call our office.
If you are more than 10 minutes past your appointment time, you may be asked to reschedule for another day appointment.
School forms:
We are happy to assist with completion of school forms for medications or action plans. Forms will be ready for pick up in one (1) week so please plan accordingly.
