What To Expect At Your Visit

Our consultation for new patients can last atleast an hour. If indicated the doctor will have you come back to the office for skin testing. *SKIN TESTING WILL NOT BE DONE AT THE FIRST VISIT* We advise against using any oral or nasal antihistamine for 5 days prior to the skin testing appointment. Patients with coughing or breathing difficulties may be asked to breath into a machine to possibly help assess breathing function. Please bring a list of all your current medications. PATIENTS UNDER THE AGE OF 18 MUST BE ACCOMPANIED BY A PARENT OR GUARDIAN. You will be asked to show your insurance card and identification at every visit. We ask that you request referrals from your primary care providers for your visits. If you are unable to keep your appointment, you must notify the office 24 hours in advance to avoid being charged. Please also understand that we accept copays prior to the visit in the form of cash, check or credit card. Please click BOTH New Patient buttons below and print and fill out the forms. Please bring BOTH packets to your visit. We look forward to seeing you!
